notice_summary_english: The subject of the order is "Successful deliveries of Continuous Ambulatory Peritoneal Dialysis (CADO) and Automatic Peritoneal Dialysis (ADO) kits for the Nephrology Clinic with the Dialysis Center of the Provincial Hospital in Kielce."
A detailed description of the subject of the order and the quantity is specified in Annex No. 2 to the SWZ - assortment and price form.
Information regarding the principles of delivery, warranty and remuneration are included in the annexes:
• appendix r 3 to the SWZ - draft provisions of the public procurement contract on the basis of a successive contract.
• Annex No. 3a - proposed provisions of the lending agreement regarding Package No. 1 and 2.
• Annex No. 3b - proposed provisions of the lease agreement in the scope of Package No. 3 and 4.
Medical devices that are the subject of the order should be understood as medical devices within the meaning of the Act of April 7, 2022 on medical devices.
Medicinal products constituting the subject of the order should be understood as medicinal products within the meaning of the Act of September 6, 2001 - Pharmaceutical Law. The offered medicinal products must be admitted to trading in accordance with the principles specified in Art. 3 or art. 4 section 1 and 2 or art. 4a of the Pharmaceutical Law Act.
PACKAGE 1 - CADO - CONTINUOUS AMBULATORY PERITONEAL DIALYSIS - number of sets 15.
PACKAGE 2 - ADO - AUTOMATIC PERITONEAL DIALYSIS - number of sets 4.
PACKAGE 3 - CADO - CONTINUOUS AMBULATORY PERITONEAL DIALYSIS - number of sets 15.
PACK 4 - ADO - AUTOMATIC PERITONEAL DIALYSIS - number of sets 10.
Date place of order execution:
a) Successive deliveries according to current needs to the delivery address each time agreed between the Parties, after submitting an appropriate order by an authorized person from the Nephrology Clinic and Dialysis Center at the expense of the Contractor from the date of conclusion of the contract for 24 months, and if the entire subject of the
