notice_summary_english: The subject of the order is "Successive supplies of drugs reimbursed under Drug Programs coded B.166 and B.161 for the Provincial Hospital in Kielce."
A detailed description of the subject of the order and the quantity is specified in Annex No. 2 to the SWZ - assortment and price form.
Information on the principles of delivery, guarantee and remuneration is included in Annex No. 3 to the SWZ - draft provisions of the public procurement contract on the basis of a successive contract.
The offered subject of the order must be consistent with the Announcement of the Minister of Health on the list of reimbursed medicines, foodstuffs for particular nutritional uses and medical devices, current as of the date of submission of offers.
Medicinal products constituting the subject of the order should be understood as medicinal products within the meaning of the Act of September 6, 2001 - Pharmaceutical Law. The offered medicinal products must be admitted to trading in accordance with the principles specified in Art. 3 or art. 4 section 1 and 2 or art. 4a of the Pharmaceutical Law Act.

TREATMENT OF PATIENTS WITH PURULATIVE APOCRINE SWEET GLANDS (HS) B.161 (ICD-10: L 73.2)

Successive deliveries according to the current needs of the Ordering Party to the Hospital Pharmacy of the Provincial Hospital in Kielce for a period of 12 months from the date of signing the contract, and if the entire subject of the order is not selected, until the assortment is exhausted according to the needs of the Ordering Party, no longer than 15 months from the date of conclusion of the contract at the prices specified in the contract.
Deliveries will be completed within 3 business days from the date of placing a written order sent by e-mail.
